NMC Issues Urgent Call for Self-Declaration Forms for New Medical Courses and Seat Increases

The National Medical Commission (NMC) has issued an urgent public notice directing medical colleges and institutions to submit self-declaration forms. This notice was issued by the NMC on April 24, 2025 based on the public notice issued by the Medical Assessment and Rating Board (MARB) dated April 23, 2025.

The notice has directed all medical colleges and institutions that have applied for the Academic Year (AY) 2025-2026 for:

  1. Starting new postgraduate (PG) medical courses
  2. Establishing new undergraduate (UG) Medical Colleges
  3. Increasing postgraduate (PG) or undergraduate (UG) seats in existing medical colleges/institutions

to submit a mandatory self-declaration form by 6:00 PM on May 9, 2025.

The notice specifies the following forms to be submitted:

I. For Starting New PG Medical Courses and Increasing PG Seats

A. Standard Assessment Form Part-B of the respective specialty. If a specialty-specific form is unavailable, Form 2.1 (Master SAF FORM-B for Broad Specialty & Super Specialty) should be used. The information declared must cover the period from April 1, 2024, to March 31, 2025.

B. Form A of the Institutional Information, which is common for all PG Specialties.

II. For Establishing New UG Medical Colleges and Increasing UG Seats:

A. Self-Declaration Form and Undertaking (For Institutions) (Form mentioned under S.No. 1.6 on the NMC website). The information declared must cover the period from April 1, 2024, to March 31, 2025.

The necessary forms are available on the NMC website under the “Information Desk > Download Application Forms” section.

The self-declaration form uploaded through the provided link must be duly filled and signed by the following authorised signatories:

  1. For UG Courses: Principal/Dean, Nodal Officer, and a minimum of 3 Heads of Departments (at least one from a clinical and one from a non-clinical department).
  2. For PG Courses: Principal/Dean, Nodal Officer, and the concerned Head of Department.

Mandatory Notarised Undertaking/Affidavit

In addition to the self-declaration form, institutions are also required to upload a notarised undertaking/affidavit on Rs. 100 stamp paper. This undertaking must also be submitted before 6:00 PM on May 9, 2025.

This urgent notice underscores the NMC’s timeline for processing applications for the upcoming academic year. Medical colleges and institutions are strongly advised to adhere to the deadline and ensure that all required forms and the notarised undertaking are submitted correctly and on time. Failure to do so may impact the processing of their applications for new courses or seat increases.
